Lyme Disease & Tick Information
How to submit a tick for testing: Please fill out the Tick Submission Form, attach your tick in a zip lock bag with your/your child's name on the bag and mail it to CAES (their address is on the bottom of the Submission Form). If you don't use the fillable format, please make sure that your email address is clearly written. You will receive results in an email from "CAES Tick Testing" (check spam folders, as well).
